Nonprofit and community organizations that need legal assistance should contact CDLC. Please note that CDLC does not provide legal assistance to individuals.

To request legal assistance from CDLC:
  • Download our Intake Questionnaire by clicking here.
  • Submit the completed questionnaire by:
    • Faxing to (317) 921-8850;
    • Mailing to CDLC at 1802 North Illinois Street, Indianapolis, IN 46202; or
    • E-mailing to info@cdlawcenter.org.
If you are unable to download the questionnaire, please call (317) 921-8806 and the Intake Questionnaire will be mailed, e-mailed, or faxed to you.
